HOW TO APPLY
- Download the membership application form from Download Form Section.
- Fill all the details in capital letters, paste one of your passport size photograph in the box given for photo, put your signature in place given for candidate signature.
- Scan the filled form in pdf format.

Membership Criteria
Type of Membership Documents Required Applicable Fee Life Membership - Copy of self-attested Diploma or B.Sc. (Anaesthesia & Operation Theatre Technology or its equivalent from a govt. recognized institute/board/university etc.) in PDF format.
- For regular govt. employee, copy of self-attested valid hospital ID-Card showing your designation in PDF format.
Rs.100/- Registration fee + Rs.300/- life membership fee= Total of Rs.400/- rupees followed by Rs.20/- rupees subscription fee from the next month onwards. Student Membership Copy of self-attested valid Student-ID card or bonafide certificate issued by your educational institution/college/university in PDF format. Rs.100/- Registration fee + Rs.150/- Student membership fee= Total of Rs.250/- followed by Rs.150/- every year from the next year till the student life. NOTE:-
- The educational institution/college/university should be Govt. approved.
- All the supporting document must be self-attested by candidate.
- Forms and documents scanned from mobile scanners are also valid.

Payment of membership fee
Payment of the member ship fee can be done by any one of the following methods:-
- Transfer the fee online through NEFT or IMPS. Write proper UTR No. or IMPS No. and date of transfer in the application form. Without this we shall be unable to track your payment. Enclose the screen shot of payment or net banking voucher with application form.
- Give the payment through
Account Payee Cheque in favour of AIAAOTT payable at New Delhi
and mention its details (ex- cheque and date) in the application form. - Give the payment By
Demand Draft drawn in favour of AIAAOTT payable at New Delhi
and mention its details (ex- draft No and date) in the application form.

Submission of Form
Submission can be done by following methods:-
- EMAIL US:- Make a folder of all your document/pdf file i.e., filled in application form, payment details, all supporting documents etc. and mail it to the following e-mail id: aiaaottformsubmission@gmail.com
- Take a print out of your filled in application form, payment details (ex-: Online transfer or account payee cheque or demand draft), Xerox copy of all the supporting documents and send it by registered post/ speed post to the following address:
